Abstract
The purpose of this research was to develop a coaching program to guide advance care planning (ACP) for older adults with dementia and their caregivers in Korea. To this end, we first analyzed five ACP guidance programs developed in other countries specifically for dementia patients and their caregivers. We identified the core components of these programs―including context, conceptual model, intervention, theory, mechanisms of action, and outcomes―using a logic model proposed by Lin et al. (2019). Programs in other countries typically focused on explaining the progression of dementia, assessing the level of understanding of dementia among patients and their caregivers, identifying the values and preferences of individuals with dementia regarding end-of-life care, and clarifying their goals of care in light of their values and preferences. Based on these findings, we developed a Korean ACP coaching program. During this process, we incorporated feedback from professionals working in dementia care settings. The core elements of the program were adapted to fit the Korean cultural context. The detailed program protocol was developed using the same logic model we used in analyzing previous research. The program we developed aims to provide support for not only individuals with dementia and their families, but also the broader older adult population, with opportunities to reflect on their values and preferences regarding end-stage dementia care and to discuss these matters with their families or care providers.
초록
본 연구는 국외 사전돌봄계획 지원 프로그램 분석을 통해 치매 또는 인지기능 저하에 대비한 사전돌봄계획의 구성요소를 제시하고, 이를 바탕으로 치매 환자 및 가족에게 도움을 줄 수 있는 한국형 사전돌봄계획 지원 프로그램을 개발하였다. 국외 프로그램들에서는 치매의 질병 진행 과정에 대한 설명, 치매 환자와 가족의 치매에 대한 이해도 파악, 임종기 돌봄과 관련한 치매 환자의 가치와 선호 파악, 치매 환자와 가족의 치료 및 돌봄 목표(goals of care)를 파악하는 것이 개입의 주요 내용인 것으로 나타났다. 이를 한국적 맥락에 적합하게 수정하고, 치매 노인 돌봄 현장 및 학계에 근무하는 전문가들의 의견을 반영하여 한국형 사전돌봄계획 지원 프로그램을 개발하였다. 구체적인 프로그램 프로토콜은 로직 모델을 활용해 제시하였다. 이러한 프로그램을 통해 초기 치매 노인과 그 가족들뿐만 아니라 일반 노인들이 치매 말기의 돌봄과 관련된 본인의 가치와 선호를 미리 생각해보고, 이를 가족 또는 돌봄제공자와 함께 이야기해볼 수 있는 기회를 제공하고자 하였다.
